What Is Behavioral Health Support?

Behavioral health support addresses the emotional, psychological, and behavioral factors that can affect a person’s overall well-being.

Care One Health takes a collaborative approach, working with patients and members of their care team to provide support based on individual needs. When appropriate, behavioral health services can be coordinated alongside primary care and other healthcare services.

Our Behavioral Health Services

Depending on the patient’s needs and clinical circumstances, support may include:

  • Behavioral Health Assessment – Our care team can help identify behavioral health concerns and determine what type of support may be appropriate.

  • Supportive Counseling – Patients may receive supportive counseling to help them navigate emotional challenges, changes in health, and other concerns affecting their well-being.

  • Psychotherapy – When appropriate, psychotherapy may be provided as part of an individual’s behavioral health treatment plan.

  • Medication Management – Medication management may be included when clinically appropriate, with coordination between behavioral health professionals and other members of the patient’s healthcare team.

  • Care Team Collaboration – Our primary care providers, licensed clinical social workers, behavioral health professionals, and other care team members can work together to support the patient’s overall health.

  • Community Resource Connections – When additional support is needed, we can help connect patients and families with appropriate community resources.
